Click here for a membership form.  This is a standard document to sign up to become a member of AWCA-TV.

Click here for the request for channel time form. This form must be filled out by the producer of the show.  Please refer to the policies handbook for details.

 •  If you are a local (Ashburnham or Westminster) producer, you will need to fill out a membership form and a channel time request form.

 •  If you are a producer from outside of Ashburnham or Westminster, you will need to find a sponsor (a person who resides in either town that is wiling to become or already is a member of AWCA-TV) to be responsible for the program to be aired in our towns. Once a sponsor has been found and becomes a member, fill out a channel time request form. All forms must be submitted to us before sending any DVDs to be aired.
