Below you will find a listing of documents and forms that you may need as a member at AWCA-TV. You can print these documents from your home computer, fill them out, and bring them in to make processes quick and efficient.


Policies Handbook - contains all our policies and regulations; a good document to be familiar with as a member to stay refreshed on our procedures

Channel Time Form - for completed programs, fill out this form to get your show aired; contains program and producer information

Series or Single Program Forms - for potential programs, fill this form out before you start your program; contains, producer information, program information, and preferred air times.

Volunteer Tracking Form - a form to track volunteers for crew calls. contains completed training, availability, and interests.

Portable equipment Request Form - This form MUST be filled out before you may take out ANY equipment.

Technical Difficulties Form - If you experience any issues with our equipment, fill this form out so we can keep track of technical difficulties.
